Health Requirements
Required Physical for Entering Grade 9
New York State mandates physicals for all students that are entering grade 9. Any physical done within one year of the start of the school is accepted. If your student does not have a current physical, the school doctor will provide one unless you opt out in writing.
The staff from the Whitney Young School Based Health Center, located in the nurse’s office on the first floor of the Watervliet Jr./Sr. High School, is available during the school day to perform yearly physicals, administer any mandated immunizations and assess any illness or injuries your student may have with no charge to you. If needed they may also assess the need for any medication your student may need to take during the school year. Your student can be brought up by bus and a parent does not have to be there. If you are interested, please call the Whitney Young School Based Health Center at (518) 629-3270 for further information.
Medication Administration at School
If your student needs to have medication administered during the school day, a written note from your student’s physician must be in place. This note must state the name of the medication, dosage and time of administration. The medication must be brought in by an adult in a clearly labelled pharmacy container with the student’s name, the name and dosage of the medication. Notes may be faxed to the High School Nurse at (518) 629-3262.
Sports Registration

Students interested in participating in varsity, junior varsity or modified athletics must complete athletic registration and submit a current physical through the district’s online registration system.
Before each athletic season, families should review eligibility requirements, registration deadlines and tryout information.
